Retirement Checklist

The following information describes the steps a member should consider taking prior to service retirement, when filing for a disability, or if terminating from MFPRSI covered employment prior to age 55.

Service Retirement

  1. Three to six months prior to your retirement date, contact the System to obtain a Service Retirement Application and a benefit estimate.
  2. Have the City complete Section D of the application.
  3. Have Section E of the application notarized.
  4. Collect the following certificates (The System reserves the right to require original or certified copies.):
    • Your birth certificate
    • Spouse’s birth certificate
    • Your marriage certificate
    • If your spouse was married prior to marriage to you, please collect your spouse’s prior marriage certificate, divorce decree, or prior spouse’s death certificate.
  5. No more than 90 days prior to your retirement date, submit the Service Retirement Application and certificates to the System.

Disability Retirement

  1. Contact the System to obtain a Disability Retirement Application.
  2. Have the City complete Section II-B of the application.
  3. Collect the following certificates (The System reserves the right to require original or certified copies.):
    • Your birth certificate
    • Spouse’s birth certificate
    • Your marriage certificate
    • If your spouse was married prior to marriage to you, please collect your spouse’s prior marriage certificate, divorce decree, or prior spouse’s death certificate.
  4. If possible, obtain copies of medical records, X-Rays, and M.R.I.’s from the physicians and clinics you’ve seen for your disabling condition. Make sure to keep these documents carefully grouped according to the physician or clinic. The System will attempt to collect this information if it is not provided with the application.
  5. Submit the Disability Retirement Application, medical records, and certificates to the System.

Termination of Employment prior to Age 55

Contact the System to discuss your options and to obtain the appropriate application for either terminated vested status or for a refund.
